Immunization
Requirements
Manatee County School Health Services
To the parents of children entering Florida schools
for the first time in School Year 2002 - 2003.
Florida laws (statutes 232.0315 and 232.032)
require that students attending Florida schools, (public,
private and charter) to have the following documentation on
file in their school health records.
1). Physical exam (DH-3040)
COMPLETED and SIGNED by a licensed physician, physician's
assistant, or ARNP. The exam must be within the 12 months
prior to your child enrolling in a Florida school. Children
are not admitted to school without this physical exam form.

2). Certificate of Immunization
(DH 680) with all immunization dates/or disease
date(s) must be completed and/or updated by the health care
provider. Students entering the designated grade levels are
required to show proof of these vaccines or a valid medical/religious
exemption before entry into school.

THESE NEW IMMUNIZATION REGULATIONS APPLY TO ALL
NEW STUDENTS & RETAINED STUDENTS.

Physical exams are available by appointment for $45.00. Immunizations
are given at no charge, Monday - Friday 8AM - 4PM; A mobile
Immunization Van is available at various sites in the community.

PRE-KINDERGARTEN
PROGRAM REQUIREMENTS: |
- DTP/DTaP/DT, Polio,
- MMR & Hib Given at appropriate
age.
- HIB For children up to 59 months
of age.
- VARICELLA 1 dose required for children
12 months and older who have not
had chicken pox disease.
- HEPATITIS B 3 dose series given for
entry or attendance to Pre-school/Day Care.

| ELEMENTARY IMMUNIZATION
REQUIREMENTS: |
- DTP/DTaP/DT 5 doses (4 doses if last
dose was given on or after the 4th birthday).
DT requires a medical statement from the doctor as
to reason why
the Pertusis vaccine was omitted.
- POLIO (OPV/IPV) 4 doses (3 doses
if last dose was given on or after the 4th birthday).
- MMR/Measles 1st dose given on or
after the 1st birthday
2nd dose given no less than one month after the first
dose
- HEPATITIS B 3-dose series given before
entry into Kindergarten
and 1st - 4th grades in School Year 2002 - 2003
- VARICELLA 1 dose required for entry
into Kindergarten and
1st grade starting the 2002 - 2003 School Year

MIDDLE AND HIGH SCHOOL IMMUNIZATION REQUIREMENTS: |
- DTP/DTaP/DT
- Polio & MMR As required for Elementary
School
- MMR/Measles 2 doses:
1st given on or after the 1st birthday
2nd dose given no less than one month after the 1st
dose
and prior to entry into 7th - 12th grades in School

2002 - 2003
Td
- (tetanus/diptheria) Has always been
recommended at least every 10 years as a booster.
Now required for 7th - 12th grades in School Year
2002 - 2003 if student
has not had a booster within previous 5 years.
- HEPATITIS B Hepatitis B Vaccine is
required for 7th - 12th grades in
School Year 2002 - 2003. Vaccine is available in either
a 2-dose or 3-dose series.

NOTE: VARICELLA
VACCINE IS RECOMMENDED FOR ANY CHILD OVER AGE 1 YEAR WHO HAS
NOT HAD CHICKEN POX DISEASE. CHILDREN WHO ARE 13 YRS OR OLDER
WOULD RECEIVE A 2-DOSE SERIES. DOCUMENTATION WITH THE YEAR
OF THE DISEASE IS REQUIRED IF VACCINE IS OMITTED.
PLEASE TAKE ALL IMMUNIZATION RECORDS
WITH YOU TO ANY DOCTOR OR CLINIC OF YOUR CHOICE. THIS WILL
HELP TO AVOID DELAYS OR
UNNECESSARY REPEATING
OF VACCINATIONS. TAKE PROOF OF PHYSICAL EXAM AND / OR IMMUNIZATIONS
RECEIVED TO THE SHCOOL YOUR CHILD WILL BE ATTENDING
